Provider network checklist
A provider directory is a useful starting point, but participation can change. Prepare your provider list offline and verify the providers and facilities that matter most through current plan and provider sources.
List privately
- Primary-care doctor and associated medical group
- Specialists, therapists, and behavioral-health providers
- Preferred hospitals, surgery centers, laboratories, and imaging centers
- Durable medical equipment and home-health suppliers
- Providers you see while traveling or living in another area
Questions to verify
- Is the specific provider—not only the practice—in network for the exact plan?
- Is the provider accepting new patients under that plan?
- Which hospitals and specialists are associated with the primary-care doctor?
- Are referrals or prior authorizations required?
- What happens for urgent, emergency, and out-of-area care?
- Could an out-of-network clinician participate in an otherwise in-network facility?
Confirm again before care
Networks may change during the year. Medicare advises checking with the provider when scheduling and contacting the plan when network status affects coverage or cost. Keep notes showing who confirmed the information and when.
