Turning 65 Medicare timeline

Your Medicare enrollment steps depend on Social Security benefits, current employment, other insurance, and when you want coverage to begin. Medicare’s Initial Enrollment Period generally lasts seven months: the three months before the month you turn 65, your birthday month, and the three months afterward.

If your birthday is on the first day of the month, Medicare uses the prior month for this timing calculation, so the seven-month period shifts one month earlier.

Six to four months before 65

  • Confirm whether Medicare enrollment will be automatic or whether you must sign up.
  • Ask your employer or benefits administrator how current coverage coordinates with Medicare.
  • Do not assume COBRA, Marketplace, retiree, or small-employer coverage creates the same Part B enrollment rights as active-employment group coverage.
  • If you contribute to an HSA, review the Medicare and HSA timing rules with a qualified benefits or tax professional.

Three months before 65

  • Your Initial Enrollment Period generally begins.
  • Confirm the desired Part A and Part B start dates.
  • Compare Original Medicare and Medicare Advantage as coverage paths.
  • If choosing Original Medicare, consider Part D and your Medigap enrollment rights and timing.

Your birthday month

If you enroll before your birthday month, coverage generally starts the month you turn 65. If you enroll during your birthday month or one of the following three months, Part B and premium-Part A generally start the next month. Special timing applies when your birthday is on the first day of a month.

After your birthday

  • Confirm that coverage and premium payments began as expected.
  • Keep enrollment confirmations, creditable drug coverage notices, and employer coverage records.
  • Review your Medicare card and plan materials for errors.
  • Schedule an annual coverage review because costs, networks, formularies, and benefits can change.

Get situation-specific assistance if you are still working, covered through a spouse, eligible before 65, living outside the United States, using VA or TRICARE benefits, enrolled in Medicaid, have ESRD, or missed an earlier enrollment period.
